Eligibility
Eligibility guidelines for Macon Volunteer Clinic include:
- Must be employed
- Live in Bibb County
- Be between the ages of 18-64
- Must be uninsured (no Medicaid, no Medicare, no ‘clinic card’ and no private or employer provided health insurance or VA benefits)
- 2009 income limits are: 1 person household – $21660; 2 person household – $29140; 3 person household – $36620; 4 person household – $44100. The income limit increases as household size increases.
- Patients must provide a valid social security number
Services
Services include physician visits, non-narcotic prescription medication, laboratory analysis, x-rays, eye exams, limited dental care and cancer screenings such as mammograms, Pap smears and colonoscopies. All services are free of charge.
Screening Hours
Patients must be screened before seeing a physician. No appointment is needed for screening on Thursday afternoons from 2:00 – 3:30p.m. Please arrive before 3:30 pm and bring the following items to the screening:
· Picture ID
· Social Security card
· Letter from your employer stating that you do work and you do not have health insurance
· If your spouse is working, a letter from your spouse’s employer stating that there is no insurance that covers you.
· If you are self employed bring your most recent 1099 or profit and loss statement for the current period
· Six payroll slips--most recent
· Two utility bills with YOUR name and address (proving that you live in Bibb County)
· Current year’s 1040 tax return. If you file separately from your spouse, we consider both incomes and you must provide the spouse's 1040 for the current year.
Appointment Hours
Appointments with a doctor are ONLY possible at these times:
· Monday 5 – 7 pm
· Tuesday 5 – 8 pm
· Wednesday 9 – 12 noon
· Thursday 9 – 12 noon
Some Saturday apppointments are available, every sixth Saturday.
Appointments with a dentist or hygeniest are ONLY possible at these times:
· Monday 1 – 4 pm
Only established patients who have seen a physician may schedule a dental appointment.
